This symptom self-check tool is intended for use by University of Oregon students, faculty, and staff. All UO community members are required to perform a symptom self-check before coming to campus or, in the case of residents, before leaving their room. Your answers will not be stored or shared; it is purely to help you monitor your health for signs of COVID-19.
When considering your symptoms, pay attention to anything you are experiencing that is outside your baseline (what is normal for you). For example, if you regularly experience migraines, you do not need to select “headache” as a symptom unless you have recently had a headache that is abnormal for you. Please note that it is possible to have COVID-19 and not experience symptoms, or to have symptoms not listed here. If you are concerned you may have COVID-19, please call your healthcare provider before visiting in person. UO students should contact University Health Services at 541-346-2770.
Additional Information for Employees: Detailed information about COVID-19 cases in the UO community is available online on the UO case tracking page. This includes information on UO employees who have been on campus during their period of transmissibility. Each employee case report includes the building where the person works, to ensure others working in that facility are made aware. Upon notification of an employee’s positive status, UO Human Resources conducts workplace contact tracing and notifies directly any other employee determined to be a close contact, informing them of the expectation to quarantine.
